The complaint must contain the following:
- Full name of person filing the complaint
- Address of person filing the complaint
- Phone number of person filing the complaint
- Email of person filing the complaint
- Age of person filing the complaint
- URL/Name of the company filing the complaint against
- If applicable the contact person at the company
- If applicable the order number of the item or service in question
- Description of the item or the service in question
- Description of the nature of the problem
- Full statement of the steps already taken to resolve the issue
- An explanation why you believe you have been treated unfairly
- A statement on what resolution you are seeking |
